Would very much appreciate a list of the shrines in MM6!

The role-playing games (I-X) that started it all and the various spin-offs (including Dark Messiah).

I've decided to start playing MM6 again, and I found an old piece of paper that I wrote a couple of years ago, anyways it contains info on which shrines one's supposed to visit each month, and their location. However the list isn't complete, could someone fill me in? The whole list would be great! Thanks in advance! :hail:
