I did struggle with this one.
Actually I wouldn't want to take a book I already know, so it might be a toss-up with the next volume of George Martin's Ice and Fire series a new Ian Rankin featuring Inspector Rebus and a new Stephen King (albeit that one might be in only because they tend to have a lot of pages; it might be a fun book, it might be a bore, so it would be a gamble to take).
For the movie, it's basically the same thing. I wouldn't want to waste my time with a movie I already know: 6th Sense sucks massively when you know the kicker. Blade Runner is one hell of a movie, but you can watch it only so often. Evil Dead is one hell of a horror movie, but I would I want to waste my time with it?
So I'd probably go for a collection of Music DVD's: Let's say Jimi Hendrix complete. An alternative would be a Jeff Beck DVD. Japan 1998 (i Think it was) or his anniversary concerts from 2003)
Music: Frank Zappa's complete works would be an option. Red Hot Chili Peppers wouldn't be bad either. Queens of the Stone Age? Iggy Pop? I have no idea. Too difficult.
However, game is easy. HoMM complete. Yup.

Book: I think I'd take Harry Potter 7 - I'd like something new to read like JJ
Music: One of my mp3 cd's or if it should be a regular one, the OST from 'La fabuleuse destin d' Amelie Poulain'
Movie: Mmm, hard to pick, I think LOTR
Game: again a though one, but I think HoMM complete would do the trick.

Book: A book I haven't read. It'd be a waste carrying on a book I already know the content. A very huge book, that is.
Record: Anything would do. I think that, for songs, photos, etc., I'd prefer remembering them as they were then just depending on a tape or else to play it henceforth. Memory should be the record.
Movie: If I happened to have a movie of my family, that'd be the first one in mind I'd take.
Game: as for game, maybe a deck of cards. I'm not attached to cards, but at least I wouldn't need a video game or computer to play it, wherever I went after leaving that island.

Let's see... I would like to take the Jan Guillou templar trilogy, but that's three books, so... perhaps something classic, like Mark Twain's unabridged collected works. That should keep me busy for a few years..
One movie? "Pirates of the Caribbean, curse of the black pearl". I could watch that twenty times without getting bored.
The record/CD... I'm thinking Sting's "Sacred Love" - it has a song for every occasion.
A game that never gets old: "Monopoly".
