How does this work in-game, are you able to select it in the starting options of a map if starting town is set to random?¿.

For me it've replaced this swamp town from HoMM with grove town. New units, buildings etc., but town replaced.

Hopefully it will look different with final release, as I've seen video on YT which suggested that...

i dont understand at all what you say

so you mean this town replaces another existing town?. And if you install this, all towns of that kind becomes replaced?
