Do the portraits change based on a choice made by the player? Or do they change with the actual hero's class?

You get the choice between these 5 types of Undeads; Necromancer, Skeleton, Ghost, Vampire and Zombie. The choice is related to the story, so you get to choose what kind of undead your hero will be transformed into. The face and class will then match your choice, like shown on the picture. So if you choose the Vampire, you'll start as a Dark Priest. Of course you can choose to change your class later on, but that won't change the picture.
If you mean how it's set up in the editor, it's basically 5 different heroes, just changed depending on the choice
